Student Health Requirements
All full-time commuter and residential students are required to have the Stevenson University Student Health Profile completed and on file in the Wellness Center by July 1 for Fall entrance and January 20 for Spring entrance. All full-time students are required to complete this profile even if they are turning in health-related records to other departments (Student Health Profile Instructions). All full time students are required to be current on MMR and Varicella vaccinations. Additionally, all students residing in SU student housing are required to obtain and provide written documentation of a meningococcal (MCV4) vaccination with their Student Health Form. If the primary dose of meningococcal vaccine was administered before the student's 16th birthday, a booster will be required before moving into residence on campus and the meningitis vaccination must not be more than five years old from the date of move-in.
Some other SU departments such as Athletics, School of Nursing and Health Professions and Stevenson University Online require health-related records to be kept on file. Despite turning in those files to the respective department, the Wellness Center still requires all full-time students to complete the Student Health Form. Please contact your individual department for their additional health-related record requirements.
Failure to submit a completed Student Health Form will block a student from registering for future classes and/or lead to removal from housing.
