Student Health Profile

Stevenson’s Student Health Profile must be completed by all new, full-time students. The profile is completed online through the Wellness Center Secure Patient Portal. Students who deposit by May 1 will have access to this portal in late May after receiving their SU single sign-on account (one log-in and password for a variety of Stevenson services). In addition to the completed health profile, all full-time students – whether residential or commuter – are required to be immunized against MMR (Measles, Mumps, and Rubella) as well as Varicella (chickenpox) according to the recommendations of the Centers for Disease Control (CDC). If immunization records are not available, students will be asked to show proof of immunity through Titer Test (blood work) results. All full-time students planning to live on-campus are also required to be immunized against Meningococcal disease.

Stevenson University highly recommends the COVID vaccine for all new students. For more information, visit the Wellness Center’s page.

There are three parts to the profile that must be completed online between late May and June 1 for fall entrance, after you have received an email with information about creating your Stevenson single sign-on account. Failure to complete the Student Health Profile by June 1 may result in delays to students moving onto campus and future class registration.

If you have any questions, please contact the Wellness Center at 443-352-4200. View our Forms page for more information.

Additional health forms are required for athletes, nursing students, and medical laboratory science students. If you have any questions, please contact the Wellness Center at 443-352-4200.
