Population-Based Care Coordination Concentration Virtual Information Session - School of Graduate and Professional Studies
Stevenson University’s new concentration within the Master of Science in Nursing program is Population-Based Care Coordination, a concept already in demand within the health care community. Nurses in multiple disciplines need to be prepared for their roles in navigating the changes ahead tied predominantly to the Affordable Care Act and changes in Maryland Health Care. Population-Based Care Coordination involves the organization of activities and information to influence the community’s overall health profile and to improve health outcomes for specific groups. This new concentration provides a foundation for nurses serving as case managers in hospital settings, community liaison nurses in insurance companies, community coordinators in health departments and more.
